Актуальные аспекты иммунопатогенеза хронического пародонтита (обзор)

Аннотация

В работе представлен анализ исследований ряда российских и зарубежных авторов, посвященных изучению иммунопатогенеза хронического пародонтита. Среди актуальных проблем современной стоматологии заболевания пародонта занимают одно из ведущих мест. Их значимость как медицинской проблемы определяется большой распространенностью различных форм патологии пародонта в мире. Пародонтит характеризуется преимущественно хроническим течением и при отсутствии своевременного лечения приводит к таким серьезным последствиям, как формирование обширных очагов одонтогенной инфекции, ослабление реактивности организма, потеря зубов и атрофия альвеолярной кости. В 2016 г. тяжелые заболевания пародонта, которые могут приводить к выпадению зубов, стали 11-ми по значимости среди распространенных в мире болезней. Большая распространенность и постоянный рост заболеваемости тканей пародонта среди населения, трудности профилактики и лечения данного заболевания поддерживают постоянный научный и практический интерес к поиску новых подходов к решению проблемы. Данные литературы об иммунологической реактивности при заболеваниях пародонта крайне разнообразны и противоречивы, что отмечают многие отечественные и зарубежные исследователи. Это может быть объяснено тем, что иммунологическая реактивность больных пародонтитом вариабельна: она зависит от степени тяжести, фазы заболевания, возраста и генетической предрасположенности пациента, типа воспалительной реакции и ряда других обстоятельств. Цель данного обзора — изучение современного понимания иммунопатогенеза хронического пародонтита с точки зрения целого комплекса этиологических патогенов.
Материалы и методы.
Проведен поиск литературы по базе PubMed (www.ncbi.nlm.nih.gov), eLibrary (elibrary.ru) и ScienceDirect (www.sciencedirect.com).
Результаты.
В результате проведенного анализа литературы большинством авторов основными этиологическими факторами признаются инфекционные агенты: не только бактерии, но и вирусы, а также грибы и их взаимодействие в процессе иммунопатогенеза хронического пародонтита, однако иммунный ответ макроорганизма определяет интенсивность течения пародонтита и выраженность деструкции тканей пародонта.
Заключение.
Понимание процессов иммунного ответа, формирования и прогрессирования пародонтита, а также выявление биомаркеров воспаления может способствовать расширению знаний о патогенетических механизмах, улучшению диагностики и поддержке различных терапевтических стратегий.
Выводы.
Исследования в области вирусных, грибковых и бактериальных пародонтологических инфекций помогут понять клинические и биологические особенности пародонтита, механизмы ответа иммунной системы и его выраженность, а также сформулировать новые стратегии борьбы с этим заболеванием. Выявление и количественная оценка пародонтальных патогенов может иметь прогностическое значение.
