Адгезивные системы: от техники тотального протравливания к универсальным адгезивам (обзор)

Аннотация

В обзорной статье дается описание истории развития адгезивных систем, приводятся актуальные классификации, химический состав и свойства данных материалов. Современные адгезивные системы различают по поколениям, технике протравливания и количеству клинических шагов. Последним поколением стоматологических адгезивов являются универсальные адгезивные системы, применение которых возможно в техниках тотального, селективного и самопротравливания. Ряд исследователей выражают сомнение в долговечности гибридного слоя и прочности сцепления данных адгезивных систем. В отечественной литературе недостаточно отражены способы улучшения и оптимизации работы с универсальными адгезивными системами.
Результаты.
По результатам анализа данных литературы (PubMed, eLibrary и ScienceDirect) все современные адгезивные системы вне зависимости от классификации имеют свои преимущества и недостатки. Адгезивные системы тотального протравливания обеспечивают высокую прочность сцепления и имеют наибольшую долговечность. Однако данные системы зависимы от степени влажности дентина, нарушение техники нанесения вызывает постоперативную чувствительность, они подвержены гидролитической деградации. Концепция самопротравливающих адгезивных систем позволяет проводить одновременное протравливание и проникновение адгезива на глубину деминерализации дентина за счет кислых мономеров. При этом низкая кислотность мономеров в составе самопротравливающего адгезива не позволяет обеспечить достаточную деминерализацию поверхности эмали. Универсальные адгезивные системы имеют уникальный состав, многофункциональность техник протравливания, низкую чувствительность при работе с ними. Они могут быть использованы при проведении как прямых, так и непрямых реставраций.
Заключение.
Обзор литературы показал, что применение универсальных адгезивных систем представляет большой практический интерес и требует дальнейшего изучения.
